CMS Special Needs Plan (SNP) Model of Care (MOC) Training

The Centers for Medicare & Medicaid Services (CMS) requires all employees, contractors, providers, and first-tier, downstream, and related entities (FDRs) who serve Medicare Advantage Special Needs Plan (SNP) members to complete annual Model of Care (MOC) training.

Purpose of MOC Training

The SNP Model of Care is a CMS-approved framework that guides how care is coordinated and delivered to SNP members, including D-SNP, C-SNP, and I-SNP populations. Training ensures an understanding of member needs, coordinated care processes, and CMS compliance requirements.

Who Must Complete Training

• Health plan employees and staff

• Contracted providers and physicians

• Care coordination and utilization management teams

• First-tier, downstream, and related entities (FDRs)

Annual Requirement

MOC training must be completed annually. Documentation of completion must be maintained for compliance and audit purposes.
