How to Read Your Wellpoint Member ID Card
Your Wellpoint member ID card includes important information you’ll need when visiting your doctor, filling prescriptions, or contacting customer service. This guide explains each section of your card so you know exactly where to find key details.
1. Member Name
Your Member Name appears near the top left of the card, usually written as:
<MemFIRST><MemLAST>
This is the official name associated with your Wellpoint health plan and should match your Medicare or insurance records.
2. Member ID Number
Look for the line labeled Member ID:
This number identifies you as a Wellpoint member.
You’ll need your Member ID whenever you:

Check in for medical appointments

Fill a prescription at the pharmacy

Contact Wellpoint Member Services

Access online services such as livehealthonline.com
3. Plan Information
Your plan name appears on the right portion of the card.
Example: Wellpoint Select (HMO)
This section tells you:

The type of Medicare Advantage plan you have

Your insurance company: Wellpoint Insurance Company

Coverage and network rules associated with your plan
4. Primary Care Physician (PCP)
The PCP: field shows the doctor assigned as your Primary Care Physician.
This is the doctor responsible for coordinating your overall care.
You may also see PCP Phone: beneath it, along with your medical group (for example, TECQ | Van Lang IPA).
5. Pharmacy (Prescription) Information
At the lower left of your card, you’ll find the fields used by pharmacies:

RxGRP

RxID
These numbers help pharmacies process your prescriptions correctly under your Wellpoint plan.
6. Copay Information
The section on the bottom right lists your copays for common services:

Preventive Copay
These are the amounts you pay at the time of service.
7. Additional Notes
Some cards include:

Tip:
Keep your Wellpoint ID card with you at all times. You’ll need it for every doctor visit, specialty appointment, and pharmacy prescription.
